Health and care organisations in Southwark and Lambeth are working together to improve the experience of patients and families as patients are discharged from hospital.
As part of this project, we want to design useful leaflets and videos to provide patients and families with information around discharge from hospital, to help them better understand the discharge process and help them know who they can contact for support both during and after discharge. We would like to design these resources together with Southwark and Lambeth residents and their families or unpaid carers.
Who can take part?
Southwark or Lambeth residents who have been discharged after an inpatient stayin the last six months from Guy’s & St Thomas’ NHS Foundation Trust, King’s College Hospital NHS Foundation Trust or South London and Maudsley NHS Foundation Trust, OR
Family members/unpaid carers who have supported a Southwark or Lambeth resident through hospital discharge in the last six months from Guy’s & St Thomas’ NHS Foundation Trust, King’s College Hospital NHS Foundation Trust or South London and Maudsley NHS Foundation Trust
What's involved?
You can attend a small group 'co-design' session, either online or in person on Wednesday, 24 July 2024:
Online session (MS Teams) at 10:30am-12:00pm
Face-to-face session (160 Tooley Street, SE1 2HZ) at 2:00pm-4:00pm
These group discussions will be facilitated by researchers from the Institute for Public Care (IPC).
A member of the team at NHS South East London London will be in touch with further information. Your personal data will not be shared with the IPC.
