The aim is to create more joined up health and care services for you in your neighbourhood.
In Greenwich we are working in four ‘neighbourhoods’ to bring local people, communities, voluntary and public sector organisations together to create a more joined up approach to health and care.
Neighbourhood working will transform how we support people’s health and wellbeing by bringing more care closer to home and making services easier to access. It will help people live healthier, more independent lives and reduce unnecessary hospital visits by providing joined-up, personalised care.

- North Central (Plumstead and Glyndon, Woolwich Arsenal, Plumstead Common, Woolwich Common, and Woolwich Dockyard)
- North East (Thamesmead Moorings, West Thamesmead, and Abbey Wood)
- South (Shooters Hill, Eltham Park and Progress, Eltham Town and Avery Hill, Mottingham, Coldharbour and New Eltham Ward, Middle Park and Horn Park, Eltham Page, Kidbrooke Villa and Sutcliffe, and Kidbrooke Park)
- West (Greenwich Creekside, Greenwich Park, East Greenwich, Greenwich Peninsula, Charlton Village and Riverside, Blackheath Westcombe, and Charlton Hornfair)
Workshops to improve services for people with long term conditions
As part of our joined up neighbourhood approach, partners are working with people and communities as well as professionals to better support people with multiple long-term conditions such as diabetes.
GPs, health and care professionals, Healthwatch and voluntary sector colleagues met in early September 2025 to hear about ambitious plans to make services more joined up for people in line with the government's 10 year plan.
They heard from a local resident, and talked about the challenges with joining up services, as well as potential solutions.
We also spoke with local people and community groups to hear about their experiences and ideas for how to improve.
A follow-on workshop in late September built on the ideas shared, and people have been involved with co-designing a new clinical pathway for people with multiple long term conditions. We'll share more details soon.
