One Bromley health and care partners are developing services for people with multiple long term health conditions as part of the integrated neighbourhoods teams (INT) programme.

Shaping the model

We invited Bromley patients with two or more long-term conditions to join us at one of our engagement events in September 2025.

The events, which included one in person and one online session, provided information about plans for a Neighbourhood Health Service and the chance to help shape a new model of care for people with multiple long term conditions in Bromley.

We focused on the following conditions:

  • Diabetes
  • Cardiovascular disease
  • Chronic kidney disease
  • Hypertension
  • Depression
  • Chronic pain
  • Respiratory disease (asthma, chronic obstructive pulmonary disease COPD)

Patients and carers feedback

The discussion allowed participants to share their lived experiences of managing these conditions, highlight what is working well and suggest where improvements could be made. The feedback gathered will play a key role in shaping how health and care services in Bromley are designed and delivered in the future.

Residents highlighted the importance of involving carers and adapting to patients’ needs and preferences. People valued the chance to be listened to and asked for follow-up and to be involved in the next steps of the project. The sessions will now inform the development of the model and how to best implement it in Bromley.

Dr Anna Robinson, GP Clinical Fellow in Long-term Conditions for Bromley said: ‘A huge thanks to everyone who gave their time and joined our sessions in person or online. They were very useful events, and the discussions and suggestions shared will help us as we develop the model in Bromley’.

Attendees at the session, fed back that ‘The discussions were run well so that everyone had the opportunity to express their views.’ and that they ‘look forward to being involved with this project as it evolves’. Another attendee shared ‘the consultation was genuine. Constructive suggestions were made by most participants.’

We thank all attendees at the events for their contributions.
