Share your views on your supported accommodation.

To make sure that the support and service provided is as good as possible, we need to ask the people who provide the service to re-apply for it every few years. You will continue to receive support no matter what you say in this survey. We would like to know what you think of it and by doing the survey now, we are working to make sure everything is done in time so there will be no break in service.

To hear from you, we have created this survey which should take about ten minutes to complete.

Your answers will be held safely and securely by the NHS to our data standards. Your responses will never be shared in a way that could identify you and your answers will not be shared outside of this project. The survey closes on Friday, 18 August 2023.

We will use your answers to help us design the supported accommodation service in Southwark from April 2024. The team running the survey are made up of representatives from NHS South East London, South London and Maudsley NHS Foundation Trust and Southwark Council who are working together in Partnership Southwark.
