The application process for the Shared care task and finish group has concluded.
Have you recently had medication prescribed or reviewed in a hospital? Would like to join a new south east London wide task and finish group focused on Shared Care between hospital and GPs?
Shared Care is an agreement between a patient, their GP and their hospital consultant. It enables information relating to the care and treatment for a specific health condition to be shared between all those parties.
We are seeking two volunteers who can provide a patient perspective to join the group. The aim is to deliver Shared Care across south east London and ensure we have the right processes in place to facilitate this process.
Other members of the group include GPs, a consultant and pharmacists across primary and secondary care in south east London.
Meetings are likely to be every other month for around 1.5 hours and will be held virtually using MS Teams for at least six months.
