Movement & Nutrition Pre-Questionnaire

To help our clinicians understand your current abilities, health status, and motivations for taking part, we ask that you complete the pre-questionnaire before starting the programme.

The information you provide will be used to assess your suitability for the programme and to help tailor support to your individual needs.

By completing and submitting this questionnaire, you consent to the clinical team reviewing relevant information contained within your health records where necessary for the purposes of your assessment and participation in the programme. All information will be handled confidentially and in accordance with data protection regulations.
