Guide to healthcare mid evaluation

This survey is for Lewisham residents and aims measure awareness and opinion about a recent NHS South East London campaign on using NHS services.

To thank you for taking part, anyone who completes the survey can be optionally entered into a prize draw to win a £50 shopping voucher – to take part you will need to share your email address with us.

The closing date for entries is the 31 March 2025. Entries received after this date will not be considered. Winners will be selected at random from all eligible entries. The winner will be contacted via email. If the winner does not respond within 30 days, we reserve the right to select an alternative winner. By entering the prize draw, participants agree to these terms and conditions.
