Guide to healthcare evaluation survey - March 2026

This survey is for south east London residents and aims to measure awareness and opinion about a recent NHS South East London campaign. 

To thank you for taking part, anyone who completes the survey can be optionally entered into a prize draw to win a £50 shopping voucher. To enter the prize draw you will need to share your email address with us. 

The closing date for entries is the 31 March 2026. Entries received after this date will not be considered. Winners will be selected at random from all eligible entries. The winner will be contacted via email. If the winner does not respond within 30 days, we reserve the right to select an alternative winner. By entering the prize draw, participants agree to these terms and conditions.
