Lived experience expression of interest form - Lewisham prevention hubs

Lewisham Health and Care Partners are looking for local residents, patients, carers and community members to help design and shape two new neighbourhood prevention hubs at two sites in the south and east of the borough, including Glass Mill Leisure Centre and Downham Leisure Centre. The hubs are being developed through a community-led co-design approach and we want local people to be at the heart of their development.  Please fill in this expression of interest from which is open until midnight on Sunday 15 November 2026.

We are not looking for formal qualifications. We are looking for a diverse group of residents representing different communities, ages, backgrounds and experiences across Lewisham, ensuring that local voices help shape the future prevention hubs so they meet the needs of all Lewisham residents.
