Three case studies: Mavis – 85+ years old Mavis has 9 long term conditions. In one year, Mavis: • had 107 total outpatient appointments, of which she attended 95 (did not attend 12 appointments) • had appointments across 10 specialties, 2 hospitals and 5 different hospital sites • attended the emergency department 4 times and had to stay in hospital for a total of 6 days over the year. Isaac – 85+ years old Isaac has 2 long term conditions In one year, Isaac: • had 270 total outpatient appointments, of which he attended 254 (did not attend 16 appointments) • had appointments across 4 specialties, 2 hospitals and 6 different hospital sites • attended the emergency department 2 times and didn’t have to stay at the hospital a single time. Ade – 50-54 years old Ade has 5 long term conditions In one year, Ade: • had 19 total outpatient appointments, of which he attended all 19 • had appointments across 11 specialties, 3 hospitals and 7 hospital sites • attended the emergency department 20 times and had to stay at the hospital for a total of 1 day.
