Consultation has concluded

A health care professional talking with a patient and his carer

Health and care partners and providers in Southwark and Lambeth are working together on a project to improve the experience of patients and families as patients are discharged from hospital.

As part of this project, we want to design useful leaflets and videos to provide patients and families with information around discharge, to help them better understand the discharge process and help them know who they can contact for support both during and after discharge. We would like to design these resources together with Southwark and Lambeth residents and their families or unpaid carers.

If you are a Southwark or Lambeth resident with an experience of hospital discharge in the last six months from Guy’s & St Thomas’ NHS Foundation Trust, King’s College Hospital NHS Foundation Trust or South London and Maudsley NHS Foundation Trust

Or

If you are a family member/unpaid carer who has recently supported a Southwark or Lambeth resident through hospital discharge in the last six months from Guy’s & St Thomas’ NHS Foundation Trust, King’s College Hospital NHS Foundation Trust or South London and Maudsley NHS Foundation Trust

We encourage you to attend one of the following co-design sessions on Wednesday, 24 July 2024:

  • Online session (MS Teams) at 10:30am-12:00pm
  • Face-to-face session (160 Tooley Street, SE1 2HZ) at 2:00pm-4:00pm

These co-design sessions will be facilitated by researchers from the Institute for Public Care (IPC). Your personal data will not be shared with the IPC.

If you are interested in attending either of these sessions, please complete this online form by Friday, 19 July 5pm: https://forms.office.com/e/fPhrCHQTDA. We will then be in touch with further information.

If you have any questions about the co-design sessions or the online form, please contact Elizabeth Skelton at Elizabeth.Skelton@selondonics.nhs.uk
