2026 rollout - Guide to healthcare baseline survey

This survey is for south east London residents and aims to measure awareness and opinion about how and when to get help from the NHS. The responses will be used as baseline results to measure the effectiveness of a communications campaign.

To thank you for taking part, anyone who completes the survey can be optionally entered into a prize draw to win a £50 shopping voucher – to take part you will need to share your email address with us. 

The closing date for entries is the 19 December 2025. Entries received after this date will not be considered. A winner will be selected at random from all eligible entries on the 21 December 2025. The winner will be contacted via email. If the winner does not respond within 30 days, we reserve the right to select an alternative winner. By entering the prize draw, participants agree to these terms and conditions.
