Guide to healthcare evaluation survey for community groups

This survey is for Lewisham residents and aims to measure awareness and opinion about a recent NHS South East London campaign. The closing date for entries is the 31 March 2025. 

To thank you for taking part, you can optionally enter into a prize draw to win a £50 shopping voucher – to take part you will need to share your email address.

Winners will be selected at random and contacted via email. If the winner does not respond within 30 days, we reserve the right to select an alternative winner. By entering the prize draw, participants agree to these terms and conditions.
