Tell us what a healthy community means to you in Plumstead and Glyndon
Plumstead and Glyndon Neighbourhood Work
Our vision for Greenwich is for everyone to be able to live a healthy, happy and independent life in a thriving community supported by joined up public services. Royal Borough of Greenwich has a commitment to community development and engagement working with our local communities to help improve health and care outcomes across the borough.
NHS South East London ICS, The Royal Borough of Greenwich and other partners are working to better connect services and community projects in Plumstead and Glyndon. This includes looking at finding solutions together through conversations with the local communities, understand what needs improving in your neighbourhood and what would help you to live a healthy life.
We are currently developing and testing a neighbourhood development approach in Plumstead and Glyndon, which aims to bring residents, patients, carers, health and care professionals (people) together as a community to design, agree and deliver the health and care they want and need. We want to ensure we engage with everyone across the community, recognising that people look for and need different things at different stages in their lives.
The aim of this initiative:
To support Primary Care Network; with an initial focus on the Plumstead and Glyndon Neighbourhood area to adopt a community development and assets-based approach to co-designing and delivering health and care working in collaboration with residents, patients, carers, voluntary sector organisations and health and care providers.
Objectives of what we want to achieve:
- To understand and connect the assets available to support the local registered population of the Primary Care Network (PCN).
- To develop a group of PCN Live Well Champions
- To embed a community connector role within the PCN.
- Utilise the unique opportunity COVID-19 has created to raise awareness and gain commitment to whole system action to address health inequalities.
- Increase engagement, co-production and co-delivery on key actions.
“Our aspiration is to work together as a community of health and care providers, residents, volunteers, patients and carers to make Plumstead and Glyndon a great place to live.”
Tell us what matters to you about the neighbourhood where you live. We want to hear from you and there are many ways you can get involved.
How you can get involved:
